Does toenail fungus spread?
Yes, toenail fungus is contagious and can spread surprisingly easily — from one nail to another, from shared household surfaces, and even through contaminated shoes, socks, nail tools, or bathroom floors. Many people don’t realize the infection is spreading until multiple nails become thickened, discolored, or damaged.
If you are unsure whether your symptoms are fungal-related, read our clinical guide on What Is Toenail Fungus? to better understand causes, progression, and treatment options.

How Contagious is Toenail Fungus
Fungi that cause toenail fungus are dermatophytes, microscopic organisms that thrive in warm, damp environments and feed on keratin, the protein that makes up nails and skin. Understanding how they spread helps explain where prevention is effective, and where it falls short.
Nail-to-Nail Direct Contact
The infection usually starts in one nail, often the big toe, and spreads to surrounding nails via direct contact. Fungal spores are easily spread when nails touch one other when walking or sleeping, or when nails are cut using poorly sterilized instruments.
If left untreated, what starts as a small yellow spot on one toenail can gradually spread to multiple nails, causing thickening, brittleness, separation from the nail bed, and persistent reinfection cycles.
Contaminated Surfaces
Dermatophyte spores can remain viable for extended periods of time on hard surfaces such as shower floors, gym locker rooms, pool decks, and shared bath mats. In these circumstances, wearing shoes is an important way to prevent infection.
All it takes is a small tear in the skin or a slight parting of the nail for the fungus to take hold. Sharing shoes and socks also significantly raises the risk of fungal transmission. Shoes that seal in heat and moisture are the ideal environment for fungus to flourish.
Autoinfection
Toenail fungus is commonly spread through repeated touching or scratching, or from poor nail hygiene. Live spores could be transferred directly by using unsterilised nail clippers on an infected nail and then on to a healthy one.
Also, towels used to dry infected feet remain a source of recontamination if not washed thoroughly between uses.
The reinfection cycle
Personal footwear is one of the most neglected causes of re-infection. Shoes and socks worn during an active infection may contain fungal spores.
Even with successful treatment the fungus might be reintroduced to newly cleansed nails by wearing the same infected footwear. This is a big reason why so many people have recurrence, especially if they solely use over-the-counter therapies and don’t address their environment.

How to Prevent Toenail Fungus from Spreading
Toenail fungus spreading prevention includes clinically recommended steps to help reduce transmission to other nails and individuals. They are most effective before infection develops or alongside prescription toenail fungus treatment

Keep Feet Clean and Dry
Wash your feet everyday with soap and water, paying special attention to the crevices between your toes, where moisture can collect. Make sure to dry properly before putting on socks, because excess moisture creates the warm environment dermatophyte fungi need to survive and multiply.
Keep nails cut short and straight, which removes locations for fungi to enter and flourish.
Avoid Sharing Personal Items
Many patients specifically ask, “Can toenail fungus spread to family members?” The answer is yes, especially when towels, showers, nail tools, socks, or bathroom floors are shared regularly inside the home.
Disinfect Footwear Regularly
Antifungal sprays or UV sanitising devices inside shoes minimize the formation of fungal matter in the long term. For people receiving treatment it is often
recommended that at the end of treatment any footwear that has been excessively worn should be replaced to prevent re-infection.
Wear Protective Footwear in Public Areas
Wearing flip-flops or waterproof sandals at gyms, pools and communal showers is a great way to cut down on exposure. Active treatment does not prevent reinfection from public surfaces leading to treatment failure.

Why Prevention Alone Isn’t Enough
Preventive measures help reduce exposure and transmission, but they cannot eliminate an active infection already living beneath the nail.
The nail plate acts as a dense keratin barrier that protects fungal organisms once they colonize the nail bed. Surface cleaning, hygiene measures, and cosmetic products cannot adequately penetrate this barrier to reach the infection at its source. As the fungus continues to grow beneath the nail, it may spread to surrounding nails, nearby skin, or other people through continued exposure.
This is exactly why many over-the-counter creams and home remedies fail. STRIDE DUO™ was designed specifically to overcome the three major barriers that make toenail fungus difficult to treat:
· The nail’s thick keratin shield, which limits topical penetration
· Protective fungal biofilm that helps organisms survive treatment
· Dormant fungal spores that contribute to recurrence and reinfection
This dual-mechanism approach combines systemic prescription therapy with advanced topical renewal technology to target fungus from both the inside and the outside simultaneously.
Prevention supports hygiene, but targeted prescription therapy is what removes the infection at its root.

What Happens If You Don’t Treat Toenail Fungus
Toenail fungus does not resolve without treatment. Over time, the infection progresses deeper beneath the nail and becomes increasingly difficult to eliminate.
Once the nail matrix becomes involved, treatment often takes longer, and permanent nail distortion may occur even after the infection is cleared.
If untreated, toenail fungus may lead to:
· Spread to neighbouring nails and surrounding skin
· Secondary bacterial infections, particularly in people with diabetes or poor circulation
· Significant nail thickening that makes future treatment more difficult
· Ongoing reinfection cycles from contaminated footwear and surfaces
Clinical research consistently shows that earlier intervention leads to faster recovery, better cosmetic outcomes, and lower recurrence rates. Delayed treatment allows the infection to become more established and harder to clear.
STRIDE DUO™ is designed to interrupt this progression early by combining prescription oral antifungal therapy with advanced topical renewal treatment to address both active fungal growth and reinfection risk.
STRIDE DUO™: Treat Toenail Fungus from the Top Down and Bottom Up
STRIDE DUO™ is a dermatologist-designed combination treatment system created to treat toenail fungus at its source beneath the nail while also improving the damaged nail surface above it.
Unlike prevention alone, STRIDE DUO™ combines prescription oral antifungal therapy with advanced topical renewal treatment in a clinically engineered “top-down + bottom-up” approach.
Prescription Oral Therapy
Prescription terbinafine or fluconazole works systemically through the bloodstream to reach the infected nail bed where topical hygiene methods cannot penetrate.
STRIDE™ also utilizes pulse dosing protocols when clinically appropriate. Rather than continuous long-term exposure, pulse dosing typically involves shorter treatment intervals, such as 7 days of medication followed by an extended period off therapy. Because terbinafine remains concentrated in nail keratin long after dosing stops, this approach may reduce overall medication exposure while continuing to target fungal growth beneath the nail.
ClearNail Renewal™
ClearNail Renewal™ works alongside oral therapy to improve topical penetration, reduce surface fungal colonisation, and support healthier nail regrowth.
Its formulation includes clinically relevant ingredients such as:
· Urea to soften and break down thickened keratin
· Lactic Acid to exfoliate damaged nail material
· Terbinafine to target fungal organisms on the nail surface
· DMSO to enhance penetration through the nail plate
Together, these ingredients help support the dual-mechanism treatment strategy used in STRIDE DUO™.
Clinician-Reviewed Treatment Online
A licensed clinician reviews your case online and determines whether prescription treatment is clinically appropriate. If approved, medications are delivered directly to your home with no in-person clinic visit required.
Clinically Guided Care
Clinical improvement and cure outcomes vary based on infection severity, treatment adherence, nail involvement, and individual response to therapy. Oral terbinafine has demonstrated strong clinical efficacy in published studies and remains one of the most widely prescribed evidence-based treatments for onychomycosis.
Whether the infection is early or advanced, STRIDE DUO™ is designed to treat the fungus at its source while helping reduce recurrence and reinfection risk.
